JKM OKU card (priority given)

IC copy (front & back)
PDF or JPG image, max 5MB
OKU card copy (if applicable)
If registered with JKM
Doctor's Support Letter / Medical Certificate
From government or private doctor
Household Income Proof
Pay slip, employer letter, or JKM Surat Akuan Miskin
Applicant Passport Photo
JPEG only, max 2MB, white background
